Important Deadlines

Graduate Student Submission Portal
The following forms should be submitted to the Graduate School via the Graduate Student Submission Portal:
- Reservation of Course for Graduate Credit
- Update of Student Academic Record (505 Form)
- Authorization to Teach Graduate Level Course
- Request for Final Oral Examination
- Change in Committee Request
- Report of Dissertation Committee (Ph.D.)
- Master’s Committee Approval
- Request for Delay of Publication
- Transfer of Credit
- Add/Drop Form: Late adds, delete drops and multiple add/drop requests only. For Q/F drops and grade status changes, please have students submit via the registrar’s Q-Drop & Grade Status Change system instead. (No Add/Drop Form required)
- Late Registration
- Leave of Absence
- Withdrawal
